Q: Does this come with a shower bracket to put it in?

It just screws onto the shower faucet that is already in.

Q: How often do you change the filter?

I only change it when it gets dark in color. Mine normally last a good month or 2!

Q: Does it work with low pressure well water?

I would say this is probably the shower head you need for that. It's by far the strongest I've ever had, but the way it puts water out, it's also soft. It also has a filter in it, which could be really useful with well water. If you order this, order a pack of filters too.

No just the shower head, and it should fit with your previous shower hose..
